LCS has announced the community recipients of its inaugural Health & Wellness Navigation Program™ Awards. The awards reflect each community’s demonstrated and elevated achievements to proactively enhance their residents' overall quality of life and well-being.  The Health & Wellness Navigation Program™ is an LCS Signature Experience for the company’s owned and managed communities.

“We know when we proactively collaborate with residents on their care coordination, care management, and well-being, we are creating an environment where residents can achieve our program goal to Live a Better Life through Better Living,” said Michele Kramer, LCS Health and Wellness Director.   “Each of these honored communities have not only created thriving communities, they are also living examples of our LCS principle to ‘serve the customer first and foremost’ through their extraordinary efforts with this program.”

The Health & Wellness Navigation Program and Health & Wellness Navigator partners with residents and families to create a unique plan that includes personalized care to maximize health and wellness goals among other individual needs for the resident.  The goal: navigate and simplify the complexities of managing and coordinating care so that we can better serve our residents.
