LCS Health Services offers a wide range of options to serve your community.

On-site visits from our expert clinical specialists support quality-of-care initiatives, clinical program implementation and identification of potential risks. When needed, additional clinical support and services are also provided.
Life Care Services provides direct access to each community’s clinical nurse specialist or clinical specialist through off-site visits. During the visits, they may perform clinical record reviews, attend meetings using video conferencing and other technologies, conduct clinical data review and analysis, make recommendations, and perform clinical risk assessments.
Our dedicated team conducts annual mock surveys with F-tags and scope and severity that replicate the Centers for Medicare & Medicaid Services process. Mock surveys are conducted ahead of CMS evaluations to help care teams prepare.
To ensure the community health center systems are staying compliant, post-acute care meetings are conducted with community leadership and the clinical nurse specialist or clinical specialist during clinical visits.
Throughout the year, the health services division provides additional education and guidance as Centers for Medicare & Medicaid Services and state governments make changes to rules, standards of care and survey processes. Our professionals use webinars, podcasts and other tools to provide timely updates to ensure the best health outcomes for residents.
Life Care Services uses Intellilogix, a robust platform from Strategic Healthcare Programs, as a vehicle for health centers to identify errors before submitting the minimum data set to the state. The portal provides access to clinical risk reports, PDPM and CMI management tools and reports, quality measures, five-star predictive reports, length of stay and rehospitalization reports and much more — all designed to manage health center operations and to help in marketing positive clinical outcomes.
Clinical specialists review plans of correction to ensure compliance with the guidelines for submission prior to sending to states after any complaint or annual survey. Health services division leadership teams work with the clinical specialists and are also available to assist with independent dispute resolution support, either on-site or virtually, and to review documentation prior to submission.
Therapy utilization remains a high priority at the Centers for Medicare & Medicaid Services. Conducting random audits for therapy compliance and utilization is an important part of a provider’s responsibility. Therapy audits provide useful information to communicate to our therapy providers to assure treatment accuracy, integrity and outcomes are supported by actual documentation.
The Centers for Medicare & Medicaid Services continues to use federal audits to assure compliance with medically necessary patient care and billing practices. RAC and ZPIC audits remain in full force. Minimum Data Set (MDS), clinical specialists and the health services division leadership are available to help with audit support when needed.

It aims to reduce anxiety and increase safety and well-being by focusing on social interactions and familiar daily activities.
The Health & Wellness Navigation Program^TM ensures residents are aware of all services and support available to them as it relates to their health. The program focuses on three key areas: care coordination, care management and health and wellness programming. Residents are aided by Health & Wellness Navigators throughout the experience.
